Survey Administration

Learn how to properly administer and verify your program's surveys.

Check For Light or Incorrect Marks

As participants complete surveys it is important to ensure that dark complete marks are made.  We recommend using blue/ black pen or a No.2 pencil to complete surveys.  Other writing instruments have varying degrees of effectiveness.  Remember: If its not in pen, fill it in!

  • Crayons

  • Colored Pencils

  • Highlighters

  • Paint​

Check For Incorrect Text/ Number Box Response

Two main field types on paper surveys are the number field and text box field.  It is imperative that all responses are inside of the response box.  Anything outside of the response box will not be read by the scanner and not included in the data.

For number fields (ex: Participant ID field), only one number can go in each box.

Old Surveys

Old surveys will not be counted.  Old surveys do not always reflect the most current survey/ target objectives.  Surveys from PY1-PY7 use the outdated Teleform software which is no longer supported by the company.  The new software can not read Teleform surveys.


If you send in old surveys, CTF Evaluation will mail them back to you and ask that you transfer the responses to the new survey. The correct surveys MUST be used this year as it is not feasible to transfer thousands of surveys internally . We have been lenient on this in the past, but will not be this year, NO EXCEPTIONS.

CTF Evaluation will mail old and unreadable surveys back to grantees a maximum of two times (per project year) as a courtesy.  If a grantee continues to submit old and/or unreadable surveys, it is the responsibility of the grantee to pick them up from our office.
